to your patients and their familiesNurse/Patient Relationship:
Develops and maintains a therapeutic relationship through the
health care continuumProvide direct patient care services to
hospice patientsEmpower patient's caregivers by education them on
care for their loved oneProvide coordination of care, ensure
continuous assessment of each patient's and family's
needsResponsible for documentation and communication of care,
medication, and observations made during care of the patient in
order to track responsiveness and health of patient.Responsible for
providing appropriate patient/family education to help
patient/family achieve a pain-free and comfortable

Vision401KPTOSCHEDULE:Monday-FridayOn-call rotation (every 12
weeks) with additional payPAY: $90,000-100,000About YOU:Registered
nurse with current licensure to practice professional nursing in
the state.Graduate of National League for Nursing accredited school
of nursing.Maintains a current CPR certification.Current valid
drivers license.At least one year of Hospice experience
(preferred)Demonstrates excellent observation, problem solving,
verbal and written communications, nursing skills per competency
checklist.Shows ability to organize and prioritize workload
independently.
