UCHealth, a leading nonprofit healthcare system based in Colorado, is seeking a skilled and compassionate Transplant Coord RN to join our dedicated team. This role is pivotal in coordinating care for transplant patients, ensuring seamless integration of services from the pre-transplant phase through post-operative care and recovery. As a Transplant Coord RN at UCHealth, you will work closely with a multidisciplinary team, providing expert nursing care and support to both patients and their families. This position requires a high level of professionalism, clinical expertise, and a commitment to providing patient-centered care consistent with our mission to improve lives and promote health. Coordinate all aspects of care for transplant patients, from initial consultation through recovery. Develop and implement care plans in collaboration with other healthcare providers. Educate patients and families about the transplant process, care, and medications. Monitor patient health and respond to clinical changes. Participate in multidisciplinary team meetings to discuss patient treatment. Maintain accurate patient records and document all care services. Ensure compliance with healthcare regulations and standards. Strong clinical skills with a focus on critical care or surgical nursing Excellent communication and interpersonal skills Ability to make quick and accurate decisions Proficient in the use of medical software for patient management Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license in the state of Colorado Certification in Clinical Transplant Coordination (CCTC) preferred
