Job Description:
The mission of the Colorado Coalition for the Homeless is to

Practitioner (NP)/Physician Assistant (PA), is responsible for
providing primary health care to individuals and families who will
be accessing care at Stout Street Health Recuperative Care Center
(SSRCC), or any of our other clinical sites. This position is for a
PRN NP/PA, mostly working weekends and holidays. The minimal
requirement is 2-10-hour shifts per month. The NP/PA would provide
high-quality acute, chronic, and preventive care across the full
age span, and works collaboratively with the SSHC multidisciplinary
integrated health team.Coalition Benefits

Functions
- Provides direct, high-quality, primary medical care to people
experiencing homelessness.
- Maintains and enhances a current level of knowledge relevant to
professional practice and continually strives to improve the
quality of care.
- Breathes the spirit of Motivational Interviewing, Harm
Reduction, and Trauma-Informed Care with clients, and provides
information and choices in a non-judgmental, respectful
manner.
- Maintains a work environment that is marked by respect for
others, that values inclusiveness and builds workforce diversity,
and that fosters cooperation and teamwork.
- Collaborates and consults effectively and bi-directionally with
all CCH staff and identifies and addresses the complex issues which
may adversely impact the health of patients.
- Maximizes community linkages while acting as advocate for
patients.
- Participates in Quality Improvement and Peer Review
activities.
- Maintains Continuing Medical Education, license (s), and CCH
Credentialing and Privileging requirements always.
- Delivers care across CCH sites which may include outpatient
clinic sites, the mobile clinic, Stout Street Recuperative Care,
street outreach or home visits. Weekend /evening/Holiday coverage
is required. Performs other duties as assigned.Qualifications
Summary

position.Requirements for Nurse Practitioners:
- A Master of Science or Doctorate degree in Nursing from a
program accredited by the American Nurse Association or the
American Association of Nurse Practitioners.
- Current registration (or ability to obtain) in the State of
Colorado as an Advanced Practice Nurse.
- Certification as a Nurse Practitioner from the American Nurse's
Credentialing Center.Requirements for Physician Assistant
- Graduation from a program accredited by the American Academy of
Physician Assistants.
- Current licensure (or ability to obtain) with the State of
Colorado to practice as a Physician Assistant.
- Current certification with the National Commission of
Certification of Physician Assistants.Department Overview &
Requirements
- Ability to prioritize multiple job responsibilities, work
independently, and exercise sound professional judgment, critical
thinking, and decision-making skills.
- Maintain an appropriate spectrum of family practice and adult
medicine experience and skills.
- Demonstrate knowledge of homeless issues. Demonstrate
sensitivity to and a desire to work with underserved
populations.
- Ability to participate as a member of a multi-disciplinary team
and function effectively with collaborating agencies.
- Bilingual preferred, not required
- 15 patient encounters per day with in a 10-hour shift$52.51 -
